Community Paramedics (CPs) will be used before an inmate is released and up to six weeks after to ensure the participant identifies and connects with a local behavioral health provider and accesses appropriate social services programs. The goal is to have the CPs meet participants in their homes within 48 hours of release to deliver a 30-day supply of medication and provide an overall health review. CPs will be on call 24-hours a day to respond to the urgent needs of the participants.

“By providing access to community paramedics, medication, and resources we believe participants’ chances of being productive members of the community will increase,” said Lance Anastasio, Chairman of the Citizens Health Care Oversite Committee. The Polk County Board of County Commissioners created the Citizens Healthcare Oversight Committee to ensure the integrity of the expenditure of the Indigent Health Care Sales Tax funds.

Certified Recovery Peer Specialists are also key team members. Each participant will be assigned a Peer Specialist to serve as mentor and advocate. Peer Specialists will help participants troubleshoot problems that may arise and encourage them to be accountable to their treatment plans.
