Duties and responsibilities:

1. Provides a supportive and therapeutic atmosphere for all individuals.
2. Assists Adult Day Services Coordinator in implementation of member’s PCP’s.
3. Assures accurate daily documentation for all members.
4. Assists members in community integration.
5. Assists in personal cares, physical needs (i.e. toileting, lifting), and behavioral issues of all members as the situation sets forth.
6. Supervises and assists members in performing such tasks as clean-up, lunch, personal care, and activities as needed.
7. Cleans and prepares program areas daily and maintains a well-organized program area.
8. Follows the assigned work schedule and never leaves members unsupervised.
9. Administers medications and maintains accurate medication documentation.
10. Responsible for daily goal and case note documentation. Utilizes proper forms and follows established procedures as outlined in Chapter 79.3 in IAC and agency documentation policy.
11. Reviews safety plans quarterly.
12. Ensures that all necessary information is provided to the ADS Coordinator and/or Team Leader regarding member issues, needs or changes in member services; addresses issues as they occur and suggests ideas for programming needs.
13. Transports members to activities of their choice.
14. Assists members in doctor-ordered therapy when needed.
15. Maintains knowledge of accreditation standards.
16. Attends ADS monthly staff meetings.
17. Attends seminars, meetings, and trainings as deemed necessary.
18. Completes necessary agency reports and paperwork in a timely manner.
19. Performs other duties as assigned.

Supervisory responsibilities:
1. All members in the Adult Day Services Program.

Knowledge, skills and abilities:

Requirements:

1. Must be 18 years of age.
2. High School Diploma or equivalent.
3. Demonstrates computer application skills.
4. Shall secure a medication manager certificate upon employment.
5. Must be able to read, speak, and write in English.
6. Holds a valid State of Iowa Driver’s License and is able to travel (must be willing to drive in urban areas to meet member needs, attend training, etc.).
7. Provides proof of valid vehicle insurance.
8. Submits to a state criminal record check. Employment is contingent upon a clear record.
9. Submits to dependent adult and child abuse record checks. Employment is contingent upon a clear record for both checks.
10. Submits to a state driver’s license check. Employment is contingent upon a clear record.
11. Must secure Class D Chauffer’s license upon hire.

Duties and responsibilities:

1. Teaches, trains, and assists members in developing the skills necessary to be as independent as possible when performing activities of daily living. These will include:
a. Training, if needed, members to take care of their own health and medical needs. Communicates any changes of these needs to Hourly Team Leader to discuss alternative methods of training and communication.
b. Assisting members to develop or maintain skills to independently access community services and to participate in their community.
c. Assisting members in developing skills to advocate for their rights and to make informed choices.
d. Assisting members in preparing an individualized diet plan to meet their nutritional needs.
e. Training members to be responsible for their safety in an emergency by conducting health and safety activities (safety drills/trainings, etc.).
f. Working with members to personally manage their personal business and property.
g. Assisting members with monthly budgeting
i. Monitoring weekly personal allowances
1. Filling out money requests
2. Ensures that receipts get turned in to the SCL Hourly Team Leader

i. Providing transportation to assist the members in obtaining services for carrying out activities of daily living such as grocery shopping, doctor appointments etc. and for community integration.
j. Providing guidance for daily living skills including cleaning, laundry, and meal preparation.
2. Assesses member’s skills and provides input for the Person-Centered Plan.
a. Communicate skills and input to Hourly Team Leader and Hourly Coordinator regarding goal suggestions
3. Completes member’s program goals and data collection related to the goals.
4. Supervises members in the care and use of clothing and personal property and assists with clothing purchases, as necessary.
5. Ensures that all persons served by the program that require 24-hour care are not left unattended.
6. Follows the assigned work schedule keeping hours scheduled to work. Contacts the SCL Hourly Department if unable to attend work in a timely manner.
7. Attends staff meetings and scheduled in-service training sessions.
8. Documents daily in PCP’s, diet menus, activities, and any other documentation necessary.
9. Ensures that SCL Hourly member medications are filled, picked up, set up, and medication goals are being completed. Administers medication correctly and maintains accurate documentation in the med logs.
10. Suggests ideas in programming to the SCL Hourly Coordinator.
11. Attends seminars, meetings, and trainings as deemed necessary.
12. Completes necessary agency reports in a timely manner.
13. Performs other duties as assigned.

1. The SCL Site Counselor supervises members when on duty at residential sites.



Physical Demands:

1. Lifts 40 lbs.
2. Obtains a physical examination from a licensed physician upon offer of employment.
