First two responses on the form are:
Name of Event - Sale-A-Brations Vendor Fair
Event Coordinator: Kingman Area Chamber of Commerce

You can fill out the form and upload it during your online registration or you can bring it into the Kingman Chamber office.

Note: If paying by cash please select the check/money order option.
Special Event Permit and all fees must be submitted 3 days prior to Vendor Fair.
